Client Forms and Application Process

Get Started

  1. You must receive a referral from one of The Mustard Seed’s partnering agencies. The  referring agency will conduct a home visit to access your furniture and housewares needs. Referrals are good for 90 days from the date of the home visit. See a list of referring agencies here: Referral Agencies General.
  2. We want to help you begin rebuilding your life as quickly as possible. To make the process go faster, please make sure you have all of the items and client forms needed to receive The Mustard Seed of Central Florida’s services. Client Checklist.
  3. Please call (407) 875-2040 to schedule your appointment. Appointments are scheduled Monday-Friday from 9 a.m. to 4 p.m.  Each client has 30 minutes in the warehouse to select their household items.  Please remember to bring picture IDs and social security cards for all adults living in the home and birth certificates for all children living in the home, your referral, client checklist, a $200 processing fee and a moving truck.
